No person, firm or corporation shall own for purposes of distribution, rental or lease to any person, firm or corporation, any juke box, amusement machine, computerized sweepstakes device, or mechanical riding device as the same are defined in this chapter for profit, without having first filed an application for a license therefore with the City Manager. Every application for such license shall be made upon a form prescribed, prepared and furnished by the City Manager, and shall set forth:
(a) The name under which the applicant conducts or intends to conduct such business;
(b) The location of the place of business;
(c) The types of machines to be sold, leased or rented to individual persons, firms or corporations;
(d) Certification from the appropriate City officials that the premises where the business is to be located complies with all zoning requirements, Building and Property Maintenance Codes, and other health and safety codes;
(e) Such other information as the City Manager may require;
(f) The application shall be signed and verified by oath or affirmation by the person owning or distributing the juke boxes or amusement machine, computerized sweepstakes device, or mechanical riding devices or in the case of an association, by a member or partner thereof, or in the case of a corporation, by an executive officer thereof, or some person specifically authorized by the corporation to sign the application, to which shall be attached the written evidence of his authority;
(g) At the time of making such application, the applicant shall pay to the City Manager a fee as hereinafter set forth.
