§ 52.05 PERMITS FOR SEWER CONNECTION SERVICES.
   (A)   Any person desiring to make connection to the municipal sewer system shall comply with all of the provisions of this chapter and shall submit an application of the person’s intentions on blanks furnished by the City Administrator/Clerk/Treasurer. The application shall be accompanied by such plans, specifications and other information as the Building Inspector or other agent of the city, together with a permit and inspection fee or such other amount as the Council may deem appropriate by resolution. All costs and expenses relating to such connection shall be borne by the applicant.
   (B)   The applicant shall, by the act of taking out such a permit, indemnify and hold harmless the city for any losses or damages which may directly or indirectly be occasioned by the installation of the sewer connection. This shall include any necessary repairs or restoration of the street or ground surfaces. A street opening fee as set forth in the annual fee schedule shall be paid at the time of application where applicable.
   (C)   Any person, firm or corporation who shall commence work of any kind for which a permit is required under this chapter without first having received such permit shall, when subsequently applying for the permit, be required to pay double the fees provided therefor and shall also be subject to any penal provisions of this chapter.
   (D)   Permits shall only be issued when the applications show that the persons, firm or corporation engaged to perform the work have been duly registered by the city to perform sewer connection installations, and who have also paid the required fee and filed the required bonds and insurance certificates indicated hereinafter. No permit shall be issued until all of the plumbing in the building has been inspected by the City Inspector and, if necessary, altered to meet standards of the state’s Plumbing Code and any other pertinent requirements of this chapter. Upon completion of the work, a copy of the permit shall be signed and dated by the licensed individual or firm making the connection and said copy shall be delivered up to the City Inspector at the time of the final inspection of the work. The City Inspector shall also sign said permit to indicate that he or she has performed an inspection and the work and materials conform with the requirements of this chapter and other regulations. The permit shall be completed by filling in the information regarding size and type of pipe used, the type of joint used, the length of the connection, the depth of the pipe at the street, the depth of the pipe at the house, the distance from either side of the house where the connection is made to the house plumbing and any other information required on the form or by the City Inspector.
   (E)   Before any person or firm is registered to engage in the business of sewer installation within the city, that person shall make application to the city and shall pay the required fee and shall file a bond, a certificate of insurance and other information as follows.
      (1)   The applicant must be a master plumber currently registered and licensed by the state or a plumber approved by the City Council; except that, such licensing requirement shall not apply to any person who shall not be involved in such sewer installation other than in the completing of the necessary excavation.
      (2)   The fee for such registration shall be set by the Building Official with the approval of the City Council.
      (3)   A certificate of insurance, acceptable to the City Council and showing coverage of not less than $50,000 per person for injuries, shall be filed. The insurance certificate shall also include provisions for accidental death to any one person and subject to the same limit for each person in an amount not less than $100,000 on account of any one accident and property damages in an amount of not less than $25,000.
   (F)   Materials and construction requirements and techniques employed by the plumber shall be subject to the approval of the Council, the City Inspector and the City Engineer, but shall, in general, comply with the following minimum requirements in addition to conforming with all of the requirements of the state’s Plumbing Code.
      (1)   All pipe shall be four-inch extra heavy cast iron soil pipe or four-inch polyvinyl chloride pipe, thick walled sewer pipe, Type W-1. If the sewer service pipe is less than ten feet from the water service pipe or if the sewer service pipe is closer than 20 feet from any well, plastic pipe shall not be allowed. In addition, no plastic pipe shall be used on unstable ground or in fill materials unless tests provided by the plumber indicate to the satisfaction of the City Council that the fill is of a density of equal or greater value than that density required to support the pipe and that the fill will be displaced at any time in the future.
      (2)   Joints shall be made air-tight using O-ring rubber gaskets or, in the case of plastic pipe, a solvent-cemented joint.
      (3)   Sewer service shall not contain more than four bends of any size angle, nor shall it contain any bend or combination of bends totaling more than 45 degrees.
      (4)   Sewer service shall not be installed parallel to any bearing wall or footing closer than three feet therefrom, nor shall it be installed with any bend closer than three feet from such wall or footing.
      (5)   Clean-outs shall be provided in any sewer service on instructions from the City Inspector, but in any case, clean-outs shall be installed at intervals of 75 feet on long services.
      (6)   Sewer service connections shall not, in any manner, be connected to, or be a part of, any septic tank or cesspool, and if any service pipe is installed across any existing septic tank or cesspool, plastic pipe shall not be allowed for that portion of the service.
      (7)   All existing septic tanks and cesspools shall be obliterated by first pumping out all organic matter and then filling with granular material to the level of the surrounding ground.
      (8)   Sewer service connections shall be made only at the location of the existing stub from the sewer main unless expressly approved by the Council to make such connection in a different location. In this case, the applicant shall bear all of the costs of making such relocated connection. Connections directly to the main sewer line shall be made with a PVC wye solvent-cemented on and held in place with two stainless steel saddle clamps.
      (9)   The minimum cover over service connections shall be six feet unless such cover is not practical at the building line, in which case the stated minimum cover shall be approved by the City Inspector prior to any work being done on the site.
