Street excavation permits shall be issued by the Director of Public Service and Safety in pursuance of a written application filed in the office of the Department. Applications and permits shall be on forms provided by the Public Service and Safety Director. The application shall give the dimensions and location of the proposed excavation, the purpose for which the excavation is made, the kind of pavement to be opened, and the approximate time when the cut will be made. Permits will be issued only for cuts to be made within one week of the date of issue. The time allowed for closing the excavation will be governed by the size and nature of the proposed
excavation.
