515.05 NOTIFICATION.
   (a)    Any company, business, organization, agency or individual which manufactures, uses, stores or otherwise handles hazardous materials shall notify the Fire Department of spills or leaks that reach 10% of the Reportable Quantities listed in Attachment 1, Table 302.4, et seq., of the Superfund Amendment and Reauthorization Act. Any material in the aforementioned lists that is airborne or has the potential to leave the site, and all fires involving such materials must be reported within five minutes of the occurrence. Whether the Fire Department responds to the incident shall be determined by the Fire Chief or his designated representative based on the nature and severity of the incident.
   (b)   Any company, business, organization, agency or individual which manufactures, uses, stores or otherwise handles hazardous materials shall submit a written report explaining the incident to the Fire Chief within one hundred and twenty hours of the occurrence of such an incident. The explanation of the incident shall include, but not be limited to:
      (1)    The name or names of the chemical or chemicals involved.
      (2)    The location of the incident.
      (3)   The apparent cause of the incident.
      (4)   Mitigation action taken by the involved reporting agency.
      (5)   Measures taken or proposed to prevent the re-occurrence of the same incident.
