1390.06 REGISTRATION REQUIREMENTS AND FORMS.
   (a)   No person shall lease, rent or cause to be occupied a Rental Dwelling Unit without first registering each Rental Dwelling Unit with the City. Registration is required to be made with the Department of Community Development by the Owner of said Rental Dwelling Unit or his/her Designated Agent through the prescribed "Rental Property Registration Form" and paying the applicable fee.
 
   (b)   Registration is required regardless of active Tenant occupancy.
 
   (c)   Upon registration and approval, the City will grant a "Rental License" permitting said Owner to rent said Rental Dwelling Unit.
 
   (d)   Application for registration of Rental Dwelling Units shall be on forms provided by the City of Springfield and shall include all information required by the City but at a minimum the following:
       (1)   Name, address, telephone, and e-mail address, if applicable, of an individual owner, sole proprietor or a corporate officer or business representative of a corporation, trust, or other entity capable of holding title; and
      (2)   Name, address, telephone, and e-mail address, if applicable, of a company or designated agent; and
      (3)   Twenty-four (24) hour emergency contact including name, address, telephone, and e-mail address, if applicable, of a company or designated agent; and
      (4)   Parcel identification number and address obtained from the (Clark County Auditor's Office).
 
   (e)   Applicants are encouraged, but not required, to provide the following information:
      (1)   Address and number of each rental dwelling unit owned in the corporate boundaries within the City of Springfield; and
      (2)   A copy of the lease used in renting the Rental Dwelling Units (template form is fine, if no lease is used, must indicate no lease); and
      (3)   Indication of whether or not the Owner accepts Vouchers from not-for-profits or other organizations providing shelter; and
      (4)   Identification of trash service provider for each Rental Dwelling Unit.
 
   (f)   No post office boxes shall be accepted as a legal address for purposes of this Chapter.
 
   (g)   Registrations shall be retained at the Community Development Office as a public record and kept by this office pursuant to the City of Springfield, Ohio's record retention schedule.
 
   (h)   Upon construction of new residential rental dwelling units or conversion of existing building space not previously used for residential rental purposes an Owner shall complete and submit a rental registration form for each building or portion thereof used for residential rental purposes.
