(A) Any elected official, part-time employee, or seasonal employee is eligible to apply for and receive medical, dental, vision, and/or life insurance coverages through the village, as outlined herein.
(B) The premium for any insurance coverage acquired by any elected official, part-time employee, or seasonal employee shall be paid by the individual at 100%, and the village shall not contribute any funds toward the various insurance premiums.
(C) All insurance coverages acquired by any elected official, part-time employee, or seasonal employee shall be paid by that individual by payroll deduction only. If the premium(s) exceed the payroll amount, insurance coverage(s) cannot be offered to that individual.
(D) Payroll deduction of insurance premiums shall be deducted as follows.
(1) Individuals paid on a quarterly basis shall have the insurance premiums deducted once per quarter for the prior three months’ insurance premiums.
(2) Individuals paid on a bi-weekly basis shall have the insurance premiums deducted the first pay period of each month for the prior month’s insurance premiums.
(3) Individuals paid on a monthly basis shall have the insurance premiums deducted once per month for the prior month’s premiums.
