§ 110.10 APPLICATION FOR LICENSE.
   (A)   Every application for a license required by this chapter shall be made in writing to the tax collector, upon a form provided by the tax collector. The application shall be signed either by the applicant or applicant's agent and shall furnish all the information as provided on the form.
   (B)   In addition to the information furnished by applicants on the form as provided in division (A) above, every solicitor applying for a license shall furnish to the tax collector the name and address of the person whom the solicitor represents, together with a description of the goods or services which the solicitor offers for sale.
