§ 95.063 IDENTIFICATION SIGNS.

   The permittee shall install permanent identification signs, at locations designated and approved by the Department of Public Works, which shall contain the following information:

   (A)   The name, address and phone number of the owner of the utility;

   (B)   The kind and nature of the utility; and

   (C)   For underground utilities, an indication of the location of the utility.
