The applicant shall state the number, location and species of trees or shrubs to be treated, sprayed, preserved, pruned, removed, cut or otherwise disturbed; the kind of treatment to be administered; the composition of the spray material to be applied; and such other information as the Director shall find reasonably necessary to a fair determination of whether a permit should be issued.
