§ 96.03 APPLICATION FOR PERMIT. 
   Any person who desires to conduct a non-transportation activity on a portion of a public right of way shall submit an application to the City Clerk on a form provided by the City at least 21 days prior to the date of the planned activity. The application shall include the following information:
   (A)   The name, address, telephone number and e-mail address of the applicant.
   (B)   The date and time of requested activity.
   (C)   A description of the portion of public right of way to be closed and used (including Map or Diagram).
   (D)    The proposed use and expected number of participants, including a detailed description of activities planned.
