A person charged with violation of a provision of this title or a traffic ordinance shall be given a notice to appear before the Municipal Court of the city at the time stated in such notice which shall be within five days from the time of the offense and that in the event of failure to do so a warrant will be issued for his or her arrest. The notice shall state the name and address of the offender, if known, the license number and make of the vehicle involved in the violation, the nature, date and location of the offense, and the time and place where the offender is to appear to answer the charge. The notice shall be signed by the police officer executing it.
