No burial in Rose Hill Cemetery or the Rose Garden will be allowed until a burial application is completed. The following information is required:
(1) Name and address of applicant;
(2) Applicant's relationship to the deceased;
(3) Name, date of birth, and date of death of the person to be buried;
(4) Block, section, lot, and space number in which burial is to be made;
(5) Name of the mortician in charge of the body; and
(6) Date and time of graveside service.
