§ 154.115 CHECKLIST FOR PRELIMINARY PLAN.
   The following is the checklist for a preliminary plan.
   (A)   __________________________________________ (Name of Subdivision);
   (B)   __________________________________________ (Date of Submission);
   (C)   ___________________________________________ (Due Date of Recommendation - 60 Days);
   (D)   (NOTE): To properly execute this checklist, the subdivider or his or her engineer shall:
      (1)   Insert the required information.
      (2)   Denote compliance with applicable ordinances by placing his or her initials in all spaces where applicable.
      (3)   Denote those items which the subdivider considers "Not Applicable" to this particular subdivision by the abbreviation "N.A.")
         (a)   ________   Six copies of preliminary plan submitted.
         (b)   ________   Plans conform to § 154.029.
         (c)   ________   Plan scale is not less than 1" to 100'.
         (d)   ________   Minimum profile scale is 1" to 100' horizontal and 1" to 10' vertical.
         (e)   ________   A title sheet is included with each set of preliminary plans.
         (f)   ________   Name of proposed subdivision shown.
         (g)   ________   Location given by town, range, section or other legal description.
         (h)   ________   Name and address of owner, trust, corporation or subdivider having control of project is shown.
         (i)   ________   Name and seal of registered engineer or surveyor who prepared topographic survey is shown.
         (j)   ________   Name and address of the designer of the plan is shown.
         (k)   ________   North direction is shown.
         (l)   ________   Date of preparation and date of revision, if any, is shown.
         (m)   ________   A location map is included indicating:
            1.   ________   A scale of not less than 1" to 1,000'.
            2.   ________   Boundary lines of adjoining land within an area bounded by the nearest arterial streets or other natural boundaries.
            3.   ________   Use of surrounding land.
            4.   ________   Ownership of the surrounding land.
            5.   ________   Alignment of existing streets.
            6.   ________   Section and corporate lines.
         (n)   ________   Boundary lines of proposed subdivision is clearly shown.
         (o)   ________   Total approximate acreage is shown.
         (p)   ________   Existing zoning classification is indicated.
         (q)   ________   The following existing items, if within the boundaries of the subdivision, or located 100' or less outside the boundaries are shown:

                  i.   ________   80' right-of-way width;

               b.   ________   Collector (secondary) streets, indicating:

               ________   Not duplicating the name of any street heretofore used in the Village or its environs, unless the street is an extension of an already existing street, in which case, the name shall be used.
            3.   ________   Street improvement plan showing location of all new street improvements, including those to the center line of previously dedicating rights-of-way, abutting the subdivision, in accordance with present standards of the Village.
            4.   ________   Utility easements:
               a.   ________   Located at the rear of each lot and other necessary locations.
               b.   ________   Not less than 10' in width on each lot.
               c.   ________   Purpose is indicated.
               d.   ________   Storm water runoff is indicated.
            5.   ________ Centerline profiles of all streets showing gradients not less than 0.4% and not more than:
               a.   ________   5.0% on collector streets;
               b.   ________   7.0% on minor streets.
            6.   ________   Pedestrian ways, when required, indicating: (See § 154.035)
               a.   ________   Location at approximately the center of the blocks in excess of 900' in length;
               b.   ________   Width not less than 10';
               3.   ________   Shrub or tree hedge at side boundary lines.
            7.   ________   Block layout, indicating: (See § 154.035)

               b.   ________   Additional access ways to parks, schools, etc., are shown in accordance with the Plan Commission's requirements;
               c.   ________   Blocks fit readily into the overall plan of the subdivision, with due consideration given to:
                  i.   ________   Topographical conditions;
                  ii.   ________   Lot planning;
                  iii.   ________   Traffic flow pattern;
                  iv.   ________   Public open space areas.
               d.   ________   Block numbers
               e.   ________   Blocks intended for commercial, industrial or institutional use are so designated.
            8.   ________   Lot layout, indicating:
               a.   ________   Lot dimensions;
               b.   ________   Lot areas, not less than those stipulated in the appropriated district regulations of the Zoning Code. (Areas may be listed by Schedule)
               c.   ________   Building setback lines shown and properly dimensioned.
               d.   _______   Proposed land use.
               e.   ________   Lot numbers.
               f.    _______   Corner lots are sufficiently larger than interior lots to allow maintenance of building setback lines on both street frontages and still allow a buildable width equal to that of the smallest interior lot in the block.
               g.   ________   All lots abut a publicly dedicated street for a distance of not less than the minimum width of the lot.
               h.   ________   Lots are as nearly rectangular in shape as is practicable.
               i.   ________   Lots are not less than the provision of the Zoning Code.
               j.   ________   Lot lines are substantially at right angles to the street lines and radial to curved street lines.
               k.   ________   Double frontage lots only where:
                  i.   ________   Lots back upon an arterial street and front on an access street;
                  ii.   ________   Topographic or other conditions make subdividing otherwise unreasonable;
                  iii.   ________   Lots can be made an additional 20' deeper than average;
                  iv.   ________   A protective screen plant-is indicated on one frontage.
               l.   ________   Lots abutting or traversed by a watercourse, drainage way, channel way, channel or stream, indicating:
                  i.   ________   Additional width and depth to provide an acceptable building site;
                  ii.   ________   Width of easement is at least 15' wider on each side of water at high water level.
               m.   ________   Due regard for natural features, such as:
                  i.   ________   Trees;
                  ii.   ________   Watercourses;
                  iii.   ________   Historic items;
                  iv.   ________   Other similar conditions.
            9.   ________   Areas intended to be dedicated for public use, indicating:
               a.   ________   Plan conforms to general development plan of the municipality;
               b.   ________   Purpose;
               c.   ________   Acreage;
            10.   ________   Source of domestic water supply, indicating:
               a.   ________   Connection to existing water mains;
               b.   ________   Location of site for community water plant.
            11.   ________   Provision for sewage disposal, indicating:
               a.   ________   Connection to existing sanitary sewer mains;
               b.   ________   Location of site for community sewage disposal plant.
            12.   ________   School sites, indicating:
               a.   ________   Location;
               b.   ________   Dimensions;
               c.   ________   Acreage.
            13.   ________   Topographic information, indicating:
               a.   ________   Proposed changes in elevation of land showing that any flooding would be relieved;
               b.   ________   Adequate installation of storm sewers would remove the possibility of flooding;
            14.   ________   Sanitary Sewer layout, indicating:
               a.   ________   Location;
               b.   ________   Size;
               c.   ________   Invert elevations at manholes;
               d.   ________   Manhole locations.
            15.   ________   Water main layout, indicating:
               a.   ________   Location;
               b.   ________   Size;
               c.   ________   Looped pattern where practicable;
               d.   ________   Fire hydrants, spaced apart not more than 400'.
            16.   ________   Storm sewer layout, indicating:
               a.   ________   Location;
               b.   ________   Catch basins not more than 600' intervals;
               c.   ________   Storm water is not carried across or around any intersection;
               d.   ________   Surface water drainage pattern for individual lot and block.
            17.   ________   Street light layout, indicating:
               a.   ________   Locations and typical street light detail, or
               b.   ________   Statement by subdivider that street lights will be installed in accordance with standards of the municipality.
         u.   ________   An outline of proposed covenants accompanies the plans, indicating the intention of the subdivider to have the covenants recorded with the final plat.
            ________   Protective against obstruction drainage easements.
         v.   ________   Typical street cross-section showing base construction, surfacing, concrete curb and sidewalk in accordance with the land improvements code.
         w.   ________   Indication that sidewalks will be installed along all lot lines coincidental with street rights-of-way.
         x.   ________   Indication on drawing or by certificate that subdivider is aware of his or her responsibility for installation of street signs and for seeding and tree planting in all parkways.
