(A) Applications for all permits required by this chapter shall be made in writing to the Mayor not less than seven days before the proposed date of the activity for which the permit is sought.
(B) Each application shall include the following information:
(1) Statement briefly describing the nature of the proposed activity;
(2) Name, address and telephone number of the person or organization wishing to conduct the activity;
(3) The date when the activity is to be conducted;
(4) The hour when the activity will start and terminate;
(5) The park or portion thereof for which the permit is desired; and
(6) An estimate of the anticipated attendance.
