§ 53.057 CLASSES OF PERMITS AND FEES.
   (A)   There shall be two classes of building sewer permits:
      (1)   For residential and commercial service; and
      (2)   For service to establishments producing industrial wastes.
   (B)   (1)   In either case, the owner or his or her agent shall make application on a special form furnished by the village. The permit application shall be supplemented by any plans, specifications or other information considered pertinent in the judgment of the village. There shall be a permit and inspection and application fee of $1,000 for residential, commercial or industrial business users. The sewer permit and inspection and application fee shall be paid to the village at the time the application is filed. Any industrial user, as a condition of obtaining a permit, must provide information describing its wastewater constituents, characteristics and type of activity. All persons desiring a sewer connection must be connected to the village water system.
      (2)   The fee for any tap on for any line larger than two-inch shall increase by $250 per half inch.
