Any person, firm, or corporation desiring to set up or offer for operation within the city any coin-operated machine or other kind of for-profit entertainment, amusement, or a combination of merchandise, entertainment, or amusement shall first make an application for a license. This application shall be in writing submitted to the City Clerk on the form approved by the City Council and shall be accompanied by an affidavit sworn to by the applicant to the effect that the operation of the coin-operated machine for which the license is desired is not prohibited by law.
