§ 51.09 SERVICES DISCONTINUED.
   (A)   A shut-off notice of no less than seven calendar days shall be served upon the occupant and owner:
      (1)   If, following a written request from the Department of Public Works, a customer fails to provide access to either a water meter; or
      (2)   If, following a written request from the Department of Public Works, a customer that is required to provide a test report on a backflow prevention device fails to timely provide such a report; or
      (3)   If all charges and penalties for water and sewerage service, or either of them, are not paid within 60 days after the due date of the bill for such service.
   (B)   The shut-off notice shall state the reason for the contemplated shut-off and the anticipated date of shut-off. In the case where shut-off is contemplated for lack of access to a water meter, the notice shall further state that access must be had for purposes of reading, inspecting or maintaining the water meter. Notice may be served upon the occupant and owner by first class mail delivery, personal delivery or by affixing the notice to the main entrance of the improvement containing the water meter.
   (C)   No water service shall be shut off without giving the customer an opportunity to have an administrative hearing before the Director of Administrative Services.
   (D)   Upon the expiration of the notice, or upon the order of the Director of Administrative Services following any administrative hearing, the Department of Public Works may disconnect service until such time as all applicable fines, fees and charges are paid and the appropriate action to be taken by the customer has been accomplished.
   (E)   Any water service discontinued for any reason under this chapter shall not be reinstated until all bills and penalties thereon are paid in full.
   (F)   The Director of Administrative Services shall have the authority to impose an adequate and sufficient cash deposit in order to provide or reinstate water service to properties which have a history of past due balances or payment defaults. The Director of Administrative Services shall issue guidelines for such deposits from time to time.
