(A) A permit is required for any pyrotechnic display. The application fee for the permit is $50 and the form for same may be obtained from the Village Administrator.
(B) The applicant must submit the completed application to the village no less than 15 days prior to the display.
(C) The applicant shall disclose the name of the licensed pyrotechnic distributor that will provide the pyrotechnic display service and the name of the licensed lead pyrotechnic operator that will conduct the display.
(D) The applicant shall provide proof of liability insurance in a sum not less than $1,000,000 with an insurance company licensed to do business in the State of Illinois and shall attach a certificate of insurance listing the village, the South Elgin and Countryside Fire Protection District, the owner of the land from which the display will be operated, and the adjacent landowners as additional insureds.
