1-8-10: DISCLOSURE STATEMENTS:
   A.   Filing Disclosure Statements Mandatory: All officers, officials, candidates for elective or appointive office and employees shall file with the village clerk a disclosure statement, which shall be in the following form:
DISCLOSURE STATEMENT
I,                              , state as follows:
Initials:
1.              I have been elected or appointed to public office by the Village of South Barrington.
OR
                I intend to run for election or be appointed to the public office of the Village of South Barrington, Illinois.
   OR
                I have been retained or employed or desire to be retained or employed by the Village of South Barrington.
   2.              I have received from the Village Clerk a copy of the Code of Ethics of the Village as set forth in Chapter 8 of Title 1 of the Village Code of the Village of South Barrington, as amended from time to time.
   3.              I understand that I have an obligation to read the Code of Ethics and the Code of Conduct of the Village.
   4.              I understand that I am subject to the provisions of the Code of Ethics of the Village.
   5.              I have read and l understand the Code of Ethics of the Village of South Barrington and, to the best of my knowledge, I am in compliance with and not in violation of the provisions of said Code:

   6.              I acknowledge and agree that upon any change of condition of any aspect of my disclosure, I am required to submit an amended Disclosure Statement within seven (7) days of any such change or event.

It is a violation of the Code of Ethics of the Village to file a false, misleading or inaccurate Disclosure Statement or to fail to file such a Statement or Amended Statement.
   B.   Filing Disclosure Statements: Disclosure statements shall be filed with the village clerk according to the following schedule:
Each official, officer and employee shall:
      1.   Annually file a disclosure statement no later than March 15 of each year.
      2.   Within ten (10) days prior to appointment or hiring of a person to serve as an official or employee.
      3.   Within twenty one (21) days after the filing of a petition for election to any office of the village.
      4.   Each official, officer and employee is required to submit an amended disclosure statement with the village within seven (7) days of any change of status.
The village clerk shall provide each person filing a disclosure statement with a written receipt indicating the date on which the disclosure statement was filed.
   C.   Availability; Copies Of Disclosure Statement: Pursuant to the terms and conditions of the freedom of information act and upon written application by any requesting party, all disclosure statements filed pursuant to this chapter shall be available to the public. (Ord. 2004-713, 5-13-2004)
