§ 114.06  PERMITS IN WRITING; SEPARATE APPLICATIONS.
   (A)   That every application for a permit to drill a well or to install a water and/or gas re-pressurizing or injection facility shall be in writing, signed by the applicant or some person duly authorized to sign on his or her behalf; and it shall be filed with the City Manager. A separate application shall be required for each well and each water and/or gas re-pressurizing or injection facility.
   (B)   The City Manager shall not accept the application for filing unless the same be accompanied with the following:
      (1)   A deposit of $500 to be applied as a permit fee, to cover cost of oil well supervision and fire prevention;
      (2)   A map or plat of the drill site showing the ownership of the land embraced therein, the exact location of the proposed well showing all buildings, property line of any parochial or public schools, hospitals, convalescent homes, orphanages or nursing homes, on the drill site, and showing all wells on adjoining drill sites and the distance thereto from the proposed location;
      (3)   Copy of approved Railroad Commission form W-1, permit to drill and any and all forms filed with the Railroad Commission regarding the well; and
      (4)   Each permittee will comply with all municipal, state and federal fire and safety guidelines prior to any drilling operations or other related operations for the drilling of any well.
