§ 115.02 APPLICATION FOR PERMIT.
   (A)   Any person, before drilling or causing to be drilled any oil or gas well within the village, shall first file an application at the Village Hall, on forms to be furnished by such person for such purposes, and show thereon the following: the name and address of the applicant; whether the applicant is an individual, partnership or corporation or limited liability company and, if a partnership or corporation or limited liability company, the names and addresses of the partners or principal officers; the date of the application; the location of the proposed well by street address and lot number; the names and addresses of the land owners on whose premises the well is to be located; and a statement of the formation proposed to be tested by such drilling. The application shall be signed by the applicant, a partner, officer or duly authorized agent.
   (B)   No well shall be commenced until the applicant or operator has been granted a permit to drill. Each well shall require a separate permit.
