Application for a driveway or sidewalk permit shall be on a prescribed form and shall be accompanied by a copy of the plans, sketches and descriptions sufficient to completely describe the proposed work. If it appears that the proposed work will be in conformity to this and all other applicable ordinances and regulations and acceptable engineering and construction procedures, and upon receipt of a permit fee and performance bond as hereinafter set forth, a permit for the proposed work shall be issued. Such applications shall be made to and filed with the Street Commissioner (General Maintenance Supervisor) at the village hall.
