Applicants for any mobile food vending license must complete and submit an application provided by the City Clerk which shall include no less than the following information and attachments:
A. Name and current address of the individual, business, or organization, to include phone and email contact information;
B. Valid and current driver's license or State-issued identification card of vendor;
C. Vehicle information relating to vehicles used in the vending, to include make, model, year, tag number;
D. Valid and current State Sales Tax License information, unless the applicant/vendor can prove to the City Clerk's satisfaction that such license is not mandated under the definition of the State Tax Code;
E. Valid County and/or State health department licensing information;
F. Nature of business and kinds of food and beverage products to be sold;
G. Length of time for which right to do business is requested;
H. A statement as to whether or not the applicant/vendor has been convicted of a felony, the nature of the offense, and the punishment or penalty assessed therefor;
I. A written, notarized statement authorizing the mobile food vendor to be present on the property of the legal owner of the land upon which a mobile vendor shall be located. Any subsequent changes in location for the mobile food vendor will require the mobile food vendor to possess a current written, notarized statement providing authorization to be present on the property of the legal owner of the land.
