All persons residing within the area in which the City of Skiatook provides ambulance service, whether in its licensed service area or the area in which the City of Skiatook is the primary provider of ambulance service, and who do not receive electric service from the City, are hereby granted the right and privilege to subscribe to such ambulance service as follows: (Ord. 2013-08, 6-11-2013)
A. By filing at annual intervals, on a form to be furnished by the City, a written request for ambulance service and payment of the sum of eighty four dollars ($84.00), or such sum as is later amended by ordinance or resolution of the City Council, which sum shall constitute the payment for the ambulance service from the date of such payment to the following June 30. The yearly subscription fee shall be due on or before July 1 of each year, and the subscription period shall end on June 30 of the following year. (Ord. 2017-04, 6-13-2017)
B. By payment of subscription fees through a third party public utility provider, upon written agreement with the City, which assesses and collects the ambulance service subscription monthly, which said monthly fee amount shall be the yearly subscription amount payable in twelve (12) equal installments, and remits the same to the City of Skiatook, as agreed between the City of Skiatook and the public utility provider.
C. The City Manager is authorized to enter into agreements with third party public utility providers to assess, collect and remit monthly ambulance service subscription fees to the City of Skiatook for ambulance service subscribers residing outside of the Municipal limits of the City of Skiatook, but who reside or whose business is located within its licensed service area or the area in which the City of Skiatook is the primary provider of ambulance service. The subscription fees may be assessed without written consent of the subscriber, but the subscriber shall be allowed to elect not to participate or "opt out" as set forth in subsection D of this section.
D. Any ambulance service subscriber located outside of the corporate or Municipal limits of the City of Skiatook and who remits subscription payments monthly pursuant to a surcharge on a third party public utility bill and who does not wish to subscribe for ambulance services, either before or after the subscription period begins, shall be entitled to elect not to participate or "opt out" on forms prescribed and approved by the City Manager and Fire Chief. Any ambulance service subscriber, not within the corporate or Municipal limits of the City of Skiatook who elects not to participate or to "opt out" of the ambulance service subscription shall not be billed the monthly subscription fee beginning with the following billing cycle for that particular utility provider, and shall be deemed to be a subscriber until the last day of the month in which the final ambulance surcharge is assessed on the utility bill.
