5-4-6: PERSONS ELIGIBLE FOR SERVICE:
The following persons ("eligible persons") are eligible to receive the supplemental ambulance service provided for in this chapter:
   A.   Residents of the City whose residence receive electric service from the City, members of their family and relatives permanently residing with such resident and who are themselves residents of the City (including their newborn children). (1991 Code § 11-16)
   B.   Persons permanently residing without the City but within the area in which the City of Skiatook provides ambulance service, whether in its licensed service area or the area in which the City of Skiatook is the primary provider of ambulance service, and have resided there for a continuous period of not less than sixty (60) days and who reside there with the permanent intention of making the same their only place of residence, and who:
      1.   Subscribe to electric service from the City; or
      2.   Who, pursuant to section 5-4-8 of this chapter, make a written election to pay for and receive supplemental ambulance service and be subject to the terms of this chapter, or who pay a monthly ambulance service surcharge at the ambulance subscription rate on a public utility bill, which said ambulance surcharge is billed and collected by the utility provider and the surcharge is remitted to the City, subject to the terms of this chapter.
   C.   Nonresidents of the City who are employed by a place of business located in the City subscribing to the City's electric service or a place of business located within the area in which the City of Skiatook provides ambulance service, whether in its licensed service area or the area in which the City of Skiatook is the primary provider of ambulance service (a "covered business"), which place of business meets the requirements of subsection B of this section when emergency ambulance service is the result of illness or injury arising out of and in the course of such nonresident's employment by a covered business and when such employee is located:
      1.   Within the corporate limits of the City; or
      2.   On the place of business located outside the Municipal limits of the City but within the area in which the City of Skiatook provides ambulance service, whether in its licensed service area or the area in which the City of Skiatook is the primary provider of ambulance service, and when such injury or medical emergency occurs at the place of business where there exists an electric meter subscribing to the electric service of the City and when the person or entity in whose name such service exists is subscribing to ambulance service of the City and is remitting the monthly ambulance service surcharge established pursuant to this chapter, or who pay a monthly ambulance surcharge at the ambulance subscription rate on a public utility bill, which said ambulance surcharge is billed and collected by the utility provider and the surcharge is remitted to the City, subject to the terms of this chapter and section 5-4-8 of this chapter, or who pursuant to section 5-4-8 of this chapter, make a written election to pay for and receive supplemental ambulance service, subject to the terms of this chapter.    D.   Any person requiring emergency ambulance service while lawfully upon the premises of any covered business as a business invitee or customer.
   E.   Residents of nursing homes, hospitals and other institutions or organizations located within the City or within five (5) miles of the geographic center of the City who subscribe to and agree to pay for ambulance service from the City as provided in section 5-4-8 of this chapter. Residents of such institutions who do not subscribe to membership in the ambulance service are not covered by the ambulance service provided by this chapter.
   F.   Persons who have no "insurance", as defined herein, but who otherwise qualify for supplementary ambulance service shall be provided such service at no cost. (1991 Code § 11-16)
