5-2-2: PERMIT REQUIREMENTS; FEES:
   A.   Permit Required: It shall be unlawful for any person who operates or who is employed by any business which produces, prepares, manufactures, sells or packages "food", as defined in section 5-2-1 of this chapter, in the city to not have in his or her possession a current and unrevoked permit approved by the health officer. (1991 Code § 11-3)
   B.   Application For Permit: Application for such a permit must be made in writing to the health officer by the person desiring such permit. Prior to approval of such application and the issuance of such permit, the health officer shall instruct such person in approved sanitation practices in food handling; provided, that the health officer is hereby authorized, in lieu of the above instruction, to require applicants for permits to attend a course of instruction in sanitary practices of food service, prior to approval of such application and issuance of such permit.    C.   Permit Fee: Upon approval of such application, the city-county health department of Tulsa County and/or Osage County shall issue said person, a permit; provided, that there shall be collected a fee as provided by the city council, but in no event shall the fee be less than six dollars ($6.00) for each permit to help defray the expenses of conducting the required course of instruction in sanitary practices of food services. Said permit shall entitle such person to work in a food establishment during the thirty six (36) months following the date on which said permit is issued. (1991 Code § 11-6; amd. 2011 Code)
   D.   Temporary Permits: The health officer is hereby authorized to issue temporary permits permitting said applicant to work as a food handler, except as described in section 5-2-3 of this chapter, until the next regular course of instruction in sanitary practices of food handling is conducted; provided, that such temporary permit shall expire on the day the above mentioned course of instruction begins.    E.   Medical Examinations: Before approval of application for a permit, the health officer may require, in his discretion, such medical and laboratory examinations as he may deem necessary to minimize the possibility of transmission of communicable diseases. (1991 Code § 11-8)
