A. Application; Information Required: At least ten (10) days prior to any activity by drilling equipment, the owner of mineral rights or his agent shall apply with the building inspector of the city for a permit to drill, on such forms as the building inspector shall provide. Forms shall include the following information:
1. Plot Plan: Plot plan containing distances from proposed wellhead to:
a. Adjoining surface property lines.
b. Streets, alleys, and/or other public/private roadways.
c. All structures, habitable or otherwise.
d. Known utility lines.
2. Proof Of Insurance: Proof of liability insurance, either specific or general in nature, insuring the driller or operator in the minimum amount of one hundred thousand dollars ($100,000.00) for drilling purposes.
3. Security Device: Proposed security device.
4. Emergency Number: A twenty four (24) hour phone number to be used for notification in case of emergency. (1991 Code § 14-273)
B. Permit Fees: A fee of ten dollars ($10.00) shall be charged to process a drilling application, and, in addition, a fee of five dollars ($5.00) for each of the following applicable inspections. All fees shall be payable at the time application is made. The following inspections shall be made, when applicable:
1. Temporary electrical service.
2. Permanent electrical service.
3. Security device installation.
4. Special inspections where necessary and appropriate. (1991 Code § 14-274)
