An application for a sewer permit shall state the location of the premises to be connected, the names of the owner and lessee, if any, the number of buildings to be connected and the roof area, also the nature of the use to which the buildings are devoted. Each application shall be signed by the licensed sewer tapper employed to do the work and by the owner of the premises and by any lessee for whose benefit the application is made or by the authorized agent or attorneys of the owner or lessee.
