949.03 GENERAL REGULATIONS; FEES.
   (a)   Each license for hauling garbage and rubbish shall be issued on an annual basis in accordance with the ordinances and resolutions of Council and contracts pursuant thereto.
   (b)   The fee for the license shall be seventy-five dollars ($75.00) per vehicle per year.
   (c)   Each licensed vehicle shall be of the packer type, or suitable for recycling in accordance with current regulations and shall be inspected annually by the Director of Public Service to determine if it is in proper working order and will not cause spillage within the Village.
   (d)   Each licensed vehicle shall be insured for a minimum amount of one million dollars ($1,000,000) for damage to property and bodily injury per accident.
   At the time of license application each prospective hauler must furnish a certificate of insurance in favor of the Village of Silver Lake evidencing coverage for vehicles and general liability. The Village of Silver Lake shall be named as an “additional insured” on the hauler(s) policy. Each applicant must further establish coverage of all its employees by the Ohio Bureau of Worker’s Compensation.
   (e)   The licensee shall provide the Village with the names and addresses of all employees working within the Village corporate limits upon request.
   (f)   All solid waste collection in the Village shall take place between the hours of 7:00 a.m. and 6:00 p.m. Residential wastes collection shall take place only on Wednesday and Friday except when a holiday occurs or for missed collections which may be picked up the following day. Commercial waste collections shall take place any day(s) Monday through Saturday. In addition, collection from residences on Kent Road and Graham Road in the Village of Silver Lake shall be limited to between the hours of 9:00 a.m. and 4:00 p.m.
   (g)   Licensed haulers shall provide a list of current customers to the Village upon request. Also, all licensed haulers shall provide recycling information on at least an annual basis upon the request of the Village. Such recycling information must be consistent with and in compliance with other governmental requirements as set forth in (h) below.
   (h)   Each prospective hauler shall be subject to current solid waste licensing requirements which may be provided by separate ordinance, or by contract pursuant to ordinance or resolution, or by administrative order of the Mayor if not otherwise provided, which requirements are incorporated herein, and which requirements shall include, but not be limited to, requiring reports covering the dump sites and recycling facilities being used, the submission of proof that the sites and facilities meet all requirements of applicable governmental regulatory entities and the submission of proof demonstrating recycling practices which reduce dumping at landfills or similar facilities. Written proof as required shall be submitted with the original application for license, each year thereafter, and at other times required by ordinance or order.
