Applicants for a commercial solid waste license, or for annual renewal thereof, shall file a city application form and the following information with the City Manager or his or her designee:
(A) The name(s), local address, permanent home and business address and local telephone number of the individual(s) applying for the license and the organization or persons on whose behalf the application is completed;
(B) The name(s), business address and telephone number of all owners, partners, general managers and principal officers;
(C) Facts demonstrating that the applicant possesses the adequate equipment and necessary qualifications to collect, transport and dispose of commercial or industrial solid waste, in a manner satisfactory to the City Manager, or his or her designee, in conformity with all applicable federal, state, county and city laws, rules and regulations;
(D) A description of all vehicles and equipment that the applicant owns or has under his control that will be used for the collection, transportation, disposal, or recycling of commercial, or industrial solid waste; the address(es) where all such vehicles will be kept and the land use classification under the City's Zoning Code of such location(s), if located in the City Limits; a declaration that the applicant has sufficient equipment in good mechanical condition to adequately conduct the business of commercial or industrial solid waste management services, if granted a license; and facts demonstrating that said vehicles and equipment conform to all applicable provisions of A.R.S. Title 28 as amended.
(E) Other information as the City Manager or his or her designee may require, to verify the applicant's qualifications and information submitted in the application. Such information may include an audited financial statement and other indicators of financial stability of the applicant;
(F) The applicant shall disclose whether he has ever had any application for a like license denied, revoked, suspended, or canceled by any public entity and the reason given therefor.
(G) The applicant shall provide proof that he has obtained all insurance as required by State law and provisions of this Chapter.
(H) The applicant shall provide the City with at least two emergency notification numbers (twenty-four-[24-] hour availability). In the event of an emergency, the Police or Fire Department may need to contact a representative of the company.
(I) The applicant will be assessed, a nonrefundable, commercial solid waste license application fee in the amount of $500;
