(A) Any person, business utility or entity of any kind desiring to excavate or work on village rights-of-way is hereby required to obtain a permit from the village and comply with all the requirements contained herein. The permit fee shall be set forth in Chapter 34 of this Code.
(B) Each application shall be accompanied by a site sketch indicating lot lines, easements, proposed layout, existing storm water management system, dimensions of proposal, dimensions from lot lines, dimensions from existing structures, dimensions from road centerline, proposed construction and proposed temporary facilities that may be required.
