Any application for a permit to solicit must contain the following information:
(A) The name of the organization with a charitable status under Section 501(c)(3) of the U.S. Internal Revenue Code that is applying for a solicitation permit;
(B) The name and address of all persons who will be engaged in solicitation on the date covered by the permit;
(C) A description of the specific location where the solicitations will occur;
(D) A disclosure of criminal charges that were made within the past five years against the organization that will be soliciting;
(E) A disclosure of any city, county, or other political subdivision where the solicitation permit applicant has solicited within the past five years; and
(F) Each application shall state the dates, not to exceed three, for which the solicitation permit is sought.
