§ 91.025 DETERMINATION OF VIOLATION.
   Whenever the Fire Chief, or his or her designee (Fire Marshal, Fire Inspector) determines by inspection that an apparent or actual violation of the Indiana General Administrative Rules, Indiana Fire Code, Indiana Building Code, Indiana Mechanical Code, Indiana Fuel Gas Code, this chapter, any other rule of the Commission or a hazardous condition exists upon any Class 1 structure within the City of Shelbyville, Indiana, the person making such determination shall issue such notice of violation or order as may be necessary for the enforcement of the Indiana General Administrative Rules, Indiana Fire Code, Indiana Building Code, Indiana Mechanical Code, Indiana Fuel Gas Code, this chapter, or any other rule of the Commission. The notice of violation shall include:
   (A)   The date of issuance;
   (B)   The name/business and address of the person charged;
   (C)   The sections of the pertinent codes which have been violated;
   (D)   Documentation of the violations;
   (E)   The range of fines which could be assessed upon continued noncompliance activity;
   (F)   The specific time allowed in which to bring the violation into compliance; and
   (G)   The name, business address, and telephone number of the official issuing the notice of violation.
