§ 35.017 PAID SICK LEAVE.
   (A)   Sick leave.
      (1)   Full-time city employees shall receive one sick day for every two months of completed service.
      (2)   Sick leave may be accumulated from year to year. Employees may not maintain an accumulated balance of over 30 sick days.
      (3)   Department heads have the discretion to request a doctor’s statement if they determine it is necessary and/or they suspect an employee is falsely calling in sick.
      (4)   Sick days may be used for the purpose of an employee’s illness or injury and for the care of an employee’s sick or injured child.
      (5)   Non-probationary employees (employees who have completed at least one year of service) who have resigned with proper notification, shall be paid in full for their accumulated sick days’ balance. Non-safety employees who have accumulated more than ten sick days at the end of a year, as specified by the Clerk-Treasurer, may have up to five sick or vacation days bought back by the city, leaving a balance of at least five days.
   (B)   Short term disability.
      (1)   Employees who become ill or injured due to non-related work circumstance may be eligible for short term disability benefits. Benefits are granted only upon the approval of the city’s short term disability administrator. Employees must apply for short term disability benefits by completing a short term disability leave of absence request form. This form must be completed and returned to the personnel department within eight days of the injury or illness. Failure to submit the STD form could result in denial of benefits. Upon approval of such benefits, there will be a one-week waiting period prior to any benefits taking effect. During this time, an employee must use five of their accrued sick, vacation and/or personal days. If the employee does not have five accrued days then this time will be without pay before short term disability takes effect. Medical documentation must be submitted to the personnel department after each doctor visit to clarify status of injury or illness. If eligible for light duty then medical doctor must submit a written list of restrictions.
      (2)   Short term disability benefits are as follows: 100% of bi-weekly base pay for days one through 90. Days 91 through 120 will be paid at 80% of the bi-weekly base pay. Days 121 through 173 will be paid at 60% of the bi-weekly base pay. Employees will have the option after 90 days to use banked times on a calendar day basis until all time is exhausted before moving to reduce pay percentage.
      (3)   Employees who are off work over 180 calendar days will be covered by the city’s long term disability plan, at which time their employment will be terminated. However, should the employee be released to work again, the city shall make an effort for re-hire.
