(A) Application for a driver’s permit shall be made in writing to the Chief of Police, which application shall contain the name, age and address of the applicant and the name of his or her employer and shall contain a statement as to whether the applicant has ever been convicted of any provision of this code, state law or city ordinance rule or regulation.
(B) The application shall be signed by the owner who shall have made reasonable determination that the driver meets the requirements of this chapter and is recommended for approval by the owner.
