1422.07 FORMS TO BE SUBMITTED BY APPLICANT.

   (a)   The following forms are attached to Ordinance 19-19 and are adopted by the City by reference as if fully set forth herein:

      (1)   Application;

      (2)   Certificate of Insurance;

      (3)   Bond Continuation Certificate; and

      (4)   Proof of Worker’s Compensation coverage or, in lieu of coverage, notarized proof of exemption status.

   (b)   These forms shall be submitted with each initial application and all renewals thereof.

   (c)   Minor non-substantive variations will be acceptable so long as the Certificate of Insurance provides certification of liability insurance of not less than fifty thousand dollars ($50,000.00); Performance bond or continuation, in the amount of ten thousand dollars ($10,000.00); Workers’ Compensation coverage not less than required by the Commonwealth of Pennsylvania; and not less than 30 days’s notice to the City of Sharon, as certificate holder, of any proposed or intended cancellation or change of limits.
