An applicant for a permit required by this chapter shall first file an application therefor, in writing, with the Code Enforcement Officer. The Code Enforcemetn Officer shall prepare and furnish forms for that purpose. Each application shall state:
(a) The location of the proposed sand blasting operation;
(b) The building or personalty or portion thereof to be sand blasted;
(c) The length of time it is estimated the job will take;
(d) The date when sand blasting work is to commence;
(e) The number of persons to be employed; and
(f) The type of equipment which will be used.
No person shall file or cause to be filed an application containing false or fraudulent statements.
