Each person proposing to construct or reconstruct sidewalks shall file an application with the City Engineer for a permit. Such application shall contain the following information:
(a) The names of the owner and the contractor;
(b) The location and frontage of the property;
(c) The character of the sidewalk to be constructed; and
(d) A statement that the sidewalk will be done in accordance with the standard specifications of the City pertaining to the construction or reconstruction of sidewalks.
If the application is in proper form, the City Engineer shall issue a permit to the applicant.
