Every landlord as defined herein shall submit to the Code Enforcement Officer a report on forms supplied by the City which include the following information as part of the license application:
(a) List of the dwelling units owned by the landlord located within the City of Sharon whether occupied or not occupied;
(b) Address of each dwelling or dwelling unit;
(c) Whether or not said dwelling or dwelling unit is occupied by a person and if so, the names of each person.
(d) The name and address and reasonable contact information of any property manager or agent employed by the landlord.
