Any fumigator, licensed in accordance with the provisions of this chapter, desiring to conduct a fumigation in any building or premises within the City shall make application to the City Clerk for a permit to do so upon blanks furnished by the City, stating the name and address of the fumigator, the location and character of the premises to be fumigated, the names of the owners or tenants and persons authorizing the work, the reason for the fumigation, the date and hours of fumigation and the general precautions to be taken to safeguard the public. Any change in time of fumigation or premises to be fumigated shall be approved in advance by the City Clerk. A permit fee of one dollar ($1.00) shall accompany each application.
