All applications for license under this chapter shall contain, at minimum the following information:
A. The name, address, social security number, birth date and telephone number of the applicant.
B. The name, address and telephone number of the applicant's employer and registered agent and state of incorporation if incorporated.
C. The nature of the business to be carried out within the village.
D. A physical description of the applicant.
E. The license number and description of any vehicle to be used in the business.
F. A statement as to whether the applicant or his employer has ever been convicted of any felony or misdemeanor, and if so, the nature of each such offense.
G. Signature of the applicant under penalty of perjury. (Ord. 371, 6-2-2015)
