11-2-5: CONTENTS OF PLATS:
Each plat shall contain the following:
   A.   Proposed name of the subdivision.
   B.   Location by section, township, range or by other legal description.
   C.   Names and addresses of the owner, subdivider, surveyor and designer of the plan.
   D.   Graphic scale.
   E.   North point.
   F.   Date of preparation.
   G.   Existing conditions in the tract and in the surrounding area to a distance of three hundred feet (300').
   H.   Boundary lines of the proposed subdivision clearly indicated.
   I.   Total approximate acreage.
   J.   Platted streets, railroad right of way and utility easements.
   K.   Boundary lines and ownership of adjoining unsubdivided land.
   L.   Sewers, water mains, culverts or other underground facilities.
   M.   Layout and width of proposed streets and utility easements showing street names, lot dimensions, parks and other public areas. The street layout shall include all contiguous land owned or controlled by the subdivider.
   N.   Proposed use of all parcels, and if a zoning change is contemplated, the proposed rezoning.
   O.   Preliminary street grades and drainage plan shall be shown on a copy of the contour map.
   P.   Data required under regulations by the county surveyor, i.e., accurate angular and linear dimensions for all lines, angles and curvatures used to describe boundaries, streets, easements and other important features.
   Q.   An identification system for all lots and blocks.
   R.   The size (in square feet) and dimensions of all lots.
   S.   Certification by a registered land surveyor to the effect that the plat represents a survey made by him and that monuments and markers thereon exist as located and that all dimensional and geodetic details are correct.
   T.   Notarized certification by the owner and by any mortgage holder of record of the adoption of the plat and the dedication of streets and other public areas.
   U.   Form for approval by the planning commission, as follows:
