The agreement referred to in § 50.026 of this code shall be substantially as follows:

This agreement entered into by and between the City of Seymour, Indiana, which shall include the Seymour Municipal Sanitation Utility, and who represent that they are the owners of the real estate situated at what is known as Lot # ___________ in Block # __________ in ____________________ Addition to _____________ in Township of _______________, Jackson County, Indiana:
Witnesseth: That for and in consideration of the issuance of a permit to tap into and connect with the Sewer and Sewer Treatment Plant, of the Seymour Municipal Sanitation Utility at the same rates and charges, which are now fixed and may hereafter be fixed by Ordinance No. 8-1977, and all ordinances and amendments which may hereafter be fixed, for residents of the City of Seymour, Indiana, the undersigned owners hereby agree, for themselves, their assigns, heirs, and legal representatives, to waive all right to remonstrate against future annexation to the City of Seymour, Indiana, and hereby consent to the extension of the boundary lines of the City of Seymour, Indiana, so as to include the property hereinabove described.
