155.05   APPLICATIONS FOR PERMITS.
A sign applicant shall make application on a form provided by the City Building Inspector. The application must be accompanied by the appropriate fee. The application shall be signed and must contain the following information:
   1.   The name, address, and telephone number of the applicant.
   2.   The location of the building, structure, or lot where the sign is to be located.
   3.   A description or map showing the position of signs in relation to nearby buildings or structures.
   4.   A blueprint or ink drawing of the plans and specifications and method of construction and attachment to the building or on the ground.
   5.   If the owner of the building, structure, or land to which or on which the sign is to be erected is not the applicant, the application must provide written consent from the owner. A copy of the lease between landlord and tenant may constitute written consent if covered by the terms of such lease.
   6.   The name of the person, firm, corporation, or association performing any work or services for compensation in connection with erecting, constructing, installing, altering, repairing, maintaining, moving, improving, demolishing, or painting any sign. All such persons shall submit in connection with the sign application proof of a bond in the amount of ten thousand dollars to be kept in full force and effect for a period of one year after completion of the proposed work. The conditions of such bond to be in compliance with all other provisions of this chapter.
   7.   Such other information as the City Building Inspector shall require to show full compliance with this and all other laws and ordinances of the City which may be applicable, including the intended duration of any temporary sign.
