(A) Application for sewer service shall be filed with the Clerk-Treasurer upon a form to be supplied by the city. The application shall state the name of the applicant and the premises to be served.
(B) All applications filed after the commencement of the operation of the system shall be accomplished by a fee of the amount that shall be set by the City Council from time to time, payable to the city, for the connection charge.
