In absence of provisions to the contrary or except as may be specifically provided elsewhere in this Code, applications for all licenses required by ordinance shall be made in writing to the Village Clerk on the form provided by the Clerk's office.
(A) The form on which any application for such license designated herein is made shall require the following information:
(1) Name of applicant.
(2) Whether the applicant is an individual, a copartnership or a corporation.
(3) If an individual, the residence, business address, telephone number, and facsimile number of the applicant.
(4) If a copartnership, the name(s), address(es), telephone numbers, and facsimile numbers of the persons entitled to share in the profits thereof.
(5) If a corporation:
(a) The state and date of incorporation.
(b) If a corporation is incorporated in a state other than the State of Illinois, the date such corporation qualified under the Illinois Business Corporation Act to transact business in Illinois.
(c) The name(s), address(es), social security numbers, telephone numbers, and facsimile numbers of all officers and directors.
(6) The number of the certificate of registration required under the Retailers' Occupation Tax Act, Service Occupation Tax Act, and/or Use Tax Act of the State of Illinois, if applicable.
(7) The location where the license is to be used, whether the location is properly zoned for its intended use, and the area of building(s) involved with this license.
(8) The type of license desired.
(9) Length of time for which such license is desired, not exceeding one (1) year.
(10) A statement as to whether the applicant has been granted in the past such a license from the Village, if none, then a statement as to such licenses granted by other municipalities in Illinois within the preceding two (2) year period; if no licenses have been granted in Illinois in the two (2) year period preceding the application, then a statement as to licenses granted by any state, other municipality, governing body or licensing authority. If ever revoked or refused to issue or renew such a license either to the applicant or to the organization he represents, and an accurate statement as to the reasons for any such revocation or refusal.
(11) Whether a surety bond, if required by other ordinance, accompanies such application at the time of filing this form.
(12) The number of vehicles used by the applicant in the conduct of its business.
(B) Each application for an initial business license shall be accompanied by the payment of a non-refundable application fee in the amount of $30, which shall be applied to the annual license fee in the event such license is issued. No such application fee shall be charged for a renewal of an existing license.
