Any person desiring a license required by § 93.021 shall make written application to the Village President and Board of Trustees, in which application shall be stated the location or proposed location of the hospital; the purpose for which it is or is intended to be built, opened, conducted or maintained; the accommodations or proposed accommodations for the inmates thereof; the nature and kind of treatment given or proposed to be given therein; the name and address of the chief physician, surgeon or attending chief physician or surgeon or board of physicians or surgeons and the owners or proposed owners thereof.
