§ 91.31 FEE.
   (A)   Applications for dog and/or cat licenses shall be made to the Village Clerk on forms provided by the Village Clerk.
   (B)   The Village Clerk shall prepare or cause to be prepared license application forms that shall afford completion of information as to the name, residence, and telephone number of each owner, along with full information as to the description and identity of each dog and cat to be licensed. For two years following the date of issuance, the Village Clerk shall keep or cause to be kept on file a copy of each application form so processed.
   (C)   Each application shall be accompanied by a current certificate of inoculation against rabies issued by the Cook County Rabies Control Officer or his deputy, or a licensed veterinarian along with the proper license fees pursuant to the following schedule:
      (1)   Annual Village resident, $5.
      (2)   Annual non-resident (dog park use), $10.00.
      (3)   Renewal application for Village resident received after June 30, $7.
   (D)   Upon receipt of a properly completed application form as well as the necessary certificate and fees as provided by this section, the Village Clerk is authorized hereby to issue such license evidenced by a tag of such size and shape as the Village Clerk shall deem expedient.
   (E)   It shall be the duty of every owner to keep and maintain a collar with the aforesaid tag securely fastened thereto around the neck of the animal so licensed, and every dog and cat found in the Village without such tag will be deemed unlicensed and shall be impounded as provided by this subchapter.
