§ 116.036 APPLICATIONS.
   (A)   Any person or entity desiring to operate a food establishment shall make written application for a Food Purveyor License to the Village Clerk on forms provided by the Regulatory Authority. An applicant shall submit an application for a Food Purveyor License at least thirty (30) calendar days before the date planned for opening a food establishment or the expiration date of the current Food Purveyor License for an existing facility.
   (B)   The application shall convey pertinent information necessary to inform the Regulatory Authority fully as to the size and nature of the place to be used for the purpose of the business and the conditions, equipment and facilities for conducting the business therein. The application shall include the applicant's full name, telephone number, and post office address; whether the applicant is an individual, firm, partnership, or corporation and if a partnership, the names of the partners together with their addresses; the name and mailing address of the proposed food establishment; the location and type of proposed food establishment; the signature of the applicant; and any other information required by the Regulatory Authority.
   (C)   If the application is for a temporary food establishment, it also shall include the inclusive dates of the proposed operation and list all of the foods to be handled and means of preparation.
   (D)   The Regulatory Authority may request other reasonable administrative information pertinent to the issuance of a Food Purveyor License.
