§ 112.005 APPLICATION FORM REQUIRED.
   An application for a Video Gaming Establishment License and a Video Gaming Terminal License shall be made to the Village on forms furnished by the Village of Schiller Park Local Liquor Control Commissioner. An application shall be filed each year. A current license holder shall file an application no later than December 1st of the year prior to the year that the new license will cover. The Application shall include:
   (A)   The name of the individual, partnership, corporation, association or other entity applying for the licenses,
   (B)   The residence, phone number and driver's license number of the applicant or partners; or, if a corporation, association or other entity, the residence, phone numbers and driver's license number of the principal officers and directors,
   (C)   The location for which the licenses are requested,
   (D)   A statement as to whether the applicant, his or her partners or the principal officers or directors of the corporation, association or other entity have been convicted of a criminal offense or ordinance violation (other than traffic or parking offenses) in any jurisdiction, and if so, a list of such convictions with date and prosecuting jurisdiction,
   (E)   A report or reports showing the applicant's gross annual sales totals and categories, including food, beverage, and alcoholic liquor sales, for the preceding twelve-month period, or for a new business, an estimate of said gross annual sales for the proceeding twelve-month period,
   (F)   The number of video gaming terminals that the applicant currently operates or proposes to operate at the licensed premises,
   (G)   A copy of the licenses issued by the State of Illinois for each video gaming terminal at that location, and a copy of the applicant's State of Illinois and Village of Schiller Park liquor license for that location,
   (H)   A site plan clearly indicating the proposed location of all video gaming terminals and the type of barrier to be utilized to segregate the area from persons under twenty-one (21) years of age if persons under twenty-one (21) years of age are allowed to enter the establishment provided, however, that such barrier shall not obstruct the view of the video gaming area which shall be visible to an employee who is over the age of twenty-one (21) at all times. Such site plan must be approved by the Local Liquor Control Commissioner of the Village of Schiller Park or a designated agent thereof, and
   (I)   Any other information required at the discretion of the Local Liquor Control Commissioner of the Village of Schiller Park.
